Guidelines to Application

Step 3:  Proof of Evidence Documents

These documents are essential for our verification process and must be certified* by an acceptable certifying person (see below).

Documents Checklist

Proof of Training & Professional Memberships

1. Certified copy of each training qualification certificate for VSR approved therapy

2. Certified copy of membership certificates for each professional organisations/associations

3. A current practitioner insurance certificate

Proof of ID

1. Certified copy of either your passport, DVLA driving licence or a recent passport-style photo, with the words “I certify this is a true likeness of the individual” written by the certifying person on the copy document.

2. A copy of a recent utility bill, not more than 6 months old

*Certified Copies

Your documents must be certified by an acceptable certifier (see below).  The certifying person should certify that the copy is “a true copy of the original” and in the case of ID documents they should state that it is “a true likeness of the individual”. 

They should ideally sign against an official stamp stating the company’s name and address. If a stamp is available or not, the individual should state their name, qualification and/or position held (for verification purposes) and date the document.

Acceptable certifying persons:  Bank Manager, Lawyer, Accountant, Notary Public, Medical Doctor, Member of the Clergy, Member of the Judiciary, Senior Civil Servant.
